Contractor/Performer Covid-19 Health Declaration
To help prevent the spread of Covid-19 in the work place every contractor or performing artist must complete and return this declaration prior to participating in events. It must be completed and returned before 08.30 on the day you intend to work. If the declaration has not been returned as requested access/entry will be refused. Should you answer YES to any of the questions below you will NOT be allowed access to work/visit. You should follow appropriate medical advice and guidelines.
